The Spillgut
It Feels Everything
“I thought it was crawling the wrong way. Like... like its belly was leading. Then the guts came out. Gods help me, the guts were looking for me.”
Among the horrors birthed in the festering tide of the Blight, few evoke as much unease as theA Spillgut.
Unlike the more common Shambler or Drowned, the Spillgut is not simply a rotting corpse driven to motion—it is a sentinel.Blightborn Thesewhose creaturesabdomen patroland Blightedlower groundstorso withhave opened into a grotesque, crawling grace, mass movingof notexposed ontissue, feet, mouths, butfluid, and grasping viscera. It is dangerous less because it pursues quickly than because everything near it becomes harder to understand, cross, and survive.

Appearance

Its passage leaves a wet, fouled trail that can spread across several yards of ground.
The Gibbering
Spillguts make almost constant noise. The sound is not a single voice but an overlapping babble of fragments: cries, laughter, choking, repeated words, wet clicks, and sounds that resemble speech without resolving into language.
People close to the creature report losing track of what they were doing, moving in the wrong direction, freezing in place, or striking at movement beside them without understanding why.
This makes the Spillgut a battlefield hazard even before it reaches anyone directly.
Distorted Ground
The exposed tissues around a Spillgut snag boots, shift under weight, and make clean movement difficult. Fighting close to one can feel less like standing beside a creature and more like standing inside the outer edge of its body.
The creature can also expel a sudden burst of pale, caustic fluid. Those struck describe intense pain and temporary blindness.
A victim brought down beside a Spillgut doesmay notbe relydragged on sight or sound to detect intruders. In combat, a Spillgut’s tendrils lash out with shocking reach, striking enemies from upSpillgut to 15remain feet away. These attacks aren’t just physical—they grapple, trip, and drag prey closer. But the true threat lies in their Screech, a pulse of Blighted resonance that empowers nearby undead with increased ferocity. Allies hit by the call lurch faster, bite harder, and resist damage longer—suggesting some kind of hive influence through resonance or shared rot memory.
If the creature is eliminated, its unnatural effect dissipates. Spillguts are relatively fragile—but allowing one to cry out even once can shift the balance of a small encounter into a nightmare.
Spillguts are susceptible to stealth-based ambushes and ranged attacks, especially when unaware. While their detection radius is wide, they often focus attention on the first intruder they detect—allowing for tactical distractions and flanks.
If caught unaware, a coordinated group may be able to kill one before it screeches, avoiding a full-blown escalation.
